How to book a seminar room?

Reservations can be made either in person at the Unit Pendidikan Berterusan, Kuala Lumpur Hospital or by telephone at extension 7371/6260. Booking Form (HKL / UPB / AK-04-01) can be downloaded from the Kuala Lumpur Hospital Intranet Portal. Seminar room booking application is within 3 months from the date of use. Bookers must complete 2 copies of the booking form within 7 days before the date of use.

What if I want to cancel the booking of an approved seminar room?

Answer 2

Bookers need to complete the Cancellation Form (HKL / UPB-03) along with the reason for cancellation and send it to the Unit Pendidikan Berterusan, Kuala Lumpur Hospital as soon as possible. The form can be downloaded from the Intranet Portal, Kuala Lumpur Hospital.

How to make an order by Kuala Lumpur Hospital outside / paid agency?

Bookers need to get approval from the HKL Director, make payment at the Unit Hasil Kuala Lumpur Hospital after getting approval and submit a copy of the payment receipt to the Unit Pendidikan Berterusan, Kuala Lumpur Hospital. Next the booking is required to complete the seminar room booking form.

Can I see the dietitian for nutrition counseling ?

Answer 1

Yes. You need to get a referral letter from the medical doctors in order to get the service from dietitian. For out patients, dietetic service is based on appointment basis in Diet Clinic.

Where is the location for Diet Clinic for out patients?

Answer 2 

Diet clinic is located at level 2 Specialist Complex and Ambulatory Care Center (SCACC).

Question 3 

Will patients be provided with enteral product (milk formula) which they used during admission upon discharge?

Answer 3 

Enteral products (milk formula) will not be provided upon discharged. However, if patients still require to continue with the enteral formula upon discharge, therefore patients will require to purchase on their own based on dietitian’s recommendation.

Are food provided in ward during my treatment and am I given a choice to choose?

Answer 4 

Yes. Food will be provided based on the orders received from ward. Menu served in wards includes standard menu or therapeutic diet based on patients’ medical condition. However, for first class patient, they are given the choice to choose based on the diet menu provided.

Can I bring my own food from outside when I am receiving treatment in ward?

Answer 5 

No. Food will be provided by the hospital based on patients’ treatments and recovery. If you would like to bring your own food from outside, you will need to get permission from the ward.

This clinic is open only to HKL’s staffs and family members. For those who are interested in joining the clinic, please refer to the nearby   Klinik Kesihatan closest to  your address.

Referral letters received at counters that specific day, will be screened by the staff-in-charge, and the appointment date will be set according to the standard procedure. Date of appointment will be informed as accordingly within 3 working days.

All calls on all matters are advised to be called based on the working hours at the Occupational Therapy Department.

Patients should get referral from a Medical Officer (from any government / private clinic or government / private hospital) /Occupational Therapist (from a government clinic / hospital) / Associate Health Officer (from a government clinic / hospital).

The house officers need to report to Unit Pengurusan Pegawai Perubatan according to the venue and time as stated in the email from Bahagian Sumber Manusia, Kementerian Kesihatan Malaysia.

- RM 30.00 for the first treatment while the second and subsequent treatments are charged according to the treatment given to the patient.

Referred for Noncitizen

- RM120.00 for patient registration and charges are also charged according to the type of treatment provided.
